Types of Palatal Expanders Explained (RPE vs. MARPE)

The truth is, the right type of palatal expander depends largely on age and stage of development. In most cases, treatment falls into two categories:

  • RPE (Rapid Palatal Expander) → for children
  • MARPE (Miniscrew-Assisted Rapid Palatal Expansion) → for teens and adults

Here’s what you need to know:

What is a Palatal Expander? 

A palatal expander is a specialized orthodontic appliance designed to gently widen the upper jaw.

By creating space at the mid-palatal suture (where the two halves of the upper jaw meet), expansion can:

  • Make room for crowded teeth
  • Improve bite alignment
  • Support better nasal breathing and airway function

The key difference between palatal expander types comes down to how they anchor and whether the jaw is still growing.

Exploring the RPE: Rapid Palatal Expander (For Children) 

Who the Rapid Palatal Expander is for:

Children and younger teens whose jaws are still developing.

How the Rapid Palatal Expander works:

An RPE is a tooth-supported device attached to the molars. As it’s gradually activated, it applies gentle pressure that allows the upper jaw to widen naturally while growth is still occurring.

Benefits of the Rapid Palatal Expander:

  • Creates space for incoming permanent teeth
  • Corrects crossbites early
  • Guides proper jaw development
  • Can reduce the need for more complex treatment later

Once growth slows and the upper jaw begins to fuse (typically in the mid-teen years), this type of expansion becomes less effective.

Exploring MARPE: Palatal Expansion for Adults & Teens

Who the MARPE appliance is for:

Older teens and adults whose upper jaws have stopped growing.

How the MARPE orthodontics works:

MARPE uses a custom appliance anchored with small titanium miniscrews (also called TADs) placed in the palate. This allows expansion forces to act directly on the bone, rather than just the teeth.

Dr. Lin personally designs and places each MARPE appliance in-office, ensuring precision and comfort throughout treatment.

What MARPE treatment looks like:

  • Custom appliance placement in our Union Square office
  • At-home activation as instructed
  • Expansion typically takes 4–8 weeks
  • Appliance remains in place for a few months for stabilization

Benefits of MARPE Orthodontics:

MARPE orthodontic treatment can:

  • Create true skeletal expansion (not just tooth movement)
  • Help correct crowding without extractions

When is MARPE Recommended?

You may be a candidate for MARPE if you:

  • Have a narrow palate or crossbite
  • Experience mouth breathing or nasal breathing issues
  • Have been told you need jaw surgery but want to explore alternatives
  • Need space for crowded teeth without removing teeth
  • Have sleep-disordered breathing concerns

Because the upper jaw is no longer growing, traditional expanders are no longer effective at this stage, making MARPE a powerful, minimally invasive option.

What if Expansion is More Complex? (DOME vs. SARPE)

In some cases, the mid-palatal suture can be more resistant to expansion. If that happens, Dr. Lin may recommend a collaborative approach:

DOME (Distraction Osteogenesis Maxillary Expansion):

DOME is a minimally invasive, in-office procedure performed with a trusted ENT or oral surgeon to help loosen the suture in order to support MARPE.

SARPE (Surgically Assisted Rapid Palatal Expansion):

SARPE is a more invasive surgical approach traditionally recommended for adults with significant skeletal limitations.

The goal is always to choose the least invasive, most effective option for your specific case.

Dr. Lin has helped many patients achieve successful expansion with MARPE alone, or with DOME support, avoiding the need for more extensive surgery.

Signs You or Your Child May Need Palatal Expansion

Signs your child may need RPE treatment:

  • Crowded or overlapping teeth
  • Crossbite
  • Mouth breathing or snoring
  • Narrow smile or high palate

Signs teens and adults may need MARPE treatment:

  • Persistent crowding or bite issues
  • Difficulty breathing through the nose
  • Narrow arches affecting smile or function
  • Being told surgery is required for correction

  • Your age and growth stage
  • Whether the palatal suture has fused
  • Severity of the jaw width discrepancy
